Creating a Personal Health Record

Keeping track of personal medical records can be a daunting task. The LIFE Center now offers instruction in how to set up a Personal Health Record.

A personal health record is a tool that enables consumers to manage health information so they can communicate more effectively and easily with health care providers. The PHR provides a convenient and confidential place to store important personal medical information. Having a current PHR will save time and minimize stress, especially in emergency situations. The LIFE Center offers an easy-to-use template as well as links to other free and commercially available PHR resources. LIFE Center Librarian Carol Stukey will provide help in creating this handy document.
