Moving a patient from one setting, like a home, hospital, or rehab setting, to another setting can be very challenging to many caregivers. Next Step in Care offers practical advice and easy-to-use printable guides that help for making smooth transitions. The website and campaign by the United Hospital Fund helps family caregivers recognize and manage transitions in care for their loved ones. The website provides boxes with drop-down menus that help caregivers select options that are specific for their family member. Included in English and Spanish are:
